Stages of pressure injury

Stage 1 : intact skin, non-blanchable redness. Stage 2 : partial-thickness loss, exposed dermis, shallow open wound or intact blister. Stage 3 : full-thickness loss, subcutaneous fat visible, no bone, tendon or muscle. Stage 4 : full-thickness loss with exposed bone, tendon or muscle. Unstageable : base covered by slough or eschar, depth cannot be assessed. Deep tissue injury : persistent non-blanchable deep red, maroon or purple discolouration. A stage is never reversed as it heals : a healing stage 3 remains a healing stage 3.
