Stages of pressure injury
Stage 1: intact skin, non-blanchable redness Stage 2: partial-thickness loss, exposed dermis, shallow open wound or intact blister Stage 3: full-thickness loss, subcutaneous fat visible, no bone, tendon or muscle Stage 4: full-thickness loss with exposed bone, tendon or muscle Unstageable: base covered by slough or eschar, depth cannot be assessed Deep tissue injury: persistent non-blanchable deep red, maroon or purple discolouration A stage is never reversed as it heals: a healing stage 3 remains a healing stage 3.
