Nasogastric tube

Verify placement before every use and every shift: aspirate gastric content and check the pH (usually 5.5 or less), and confirm by x-ray after insertion or if there is any doubt. Auscultating an air bolus is not a valid verification. Measure and record drainage. Green drainage is normal gastric or bilious content; coffee-ground drainage suggests bleeding. Irrigate as ordered, keep the head of bed elevated at least 30 degrees, provide mouth and nostril care, and check the skin under the securing device.
