Clozapine — what to monitor
Agranulocytosis: mandatory blood count protocol. Sore throat, fever or flu-like symptoms mean an urgent complete blood count and withholding the dose pending results. Myocarditis: chest pain, dyspnea, tachycardia, especially in the first two months. Seizures: dose-related. Severe constipation, up to bowel obstruction: monitor transit actively, this is a real cause of death. Hypersalivation, sedation, weight gain, metabolic syndrome. Never restart after an interruption without re-titrating.
