Neurological assessment
Level of consciousness first: it changes before anything else. Orientation to person, place, time and situation. Pupils: size, equality, reaction to light. Compare with the person's own baseline. Motor: strength in the four limbs, symmetry, pronator drift. Sensation, speech, ability to follow commands. Vital signs, including the pulse pressure. Compare with the previous assessment, not with a textbook. The trend matters more than any single value.
