Extrapyramidal effects
Acute dystonia: sustained contraction, torticollis, oculogyric crisis, jaw or tongue spasm. Hours to days after starting. Treat with an anticholinergic. Watch the airway. Akathisia: inner restlessness, irresistible need to move, usually in the legs. Days to weeks. Often mistaken for anxiety; raising the antipsychotic makes it worse. Parkinsonism: tremor, rigidity, bradykinesia, shuffling gait. Weeks. Tardive dyskinesia: involuntary movements of the tongue, face and limbs. Months to years, often irreversible.
